If you have a long-lasting illness that has persisted for six months or more, you may benefit from a Chronic Disease Management Plan, also known as a Care Plan. Chronic conditions can gradually worsen over time, impacting your daily life and reducing your ability to stay independent.

What is a Care Plan?

A Care Plan is an annual, personalized healthcare plan designed to help you manage your chronic illness effectively. Developed in collaboration with your GP and up to 5 allied health providers—such as dieticians, physiotherapists, podiatrists, psychologists, and occupational therapists—the plan ensures comprehensive care tailored to your needs. Many of these services are partially or fully subsidized by Medicare.

How to get started

To get a Care Plan, you'll need a consultation with your regular GP, where your health needs will be discussed, and the plan will be created. You can easily book your consultation online, over the phone, or in person.
