Chronic Disease Management (CDM) plans are designed to help patients living with long- term health conditions better manage their care and stay on top of their health goals.

GPs consulting from our clinic can work with eligible patients to create personalised care plans tailored to their individual needs. These plans may also include referrals to Allied Health Practitioners such as physiotherapists, podiatrists, dietitians, or psychologists — with some sessions potentially covered by Medicare.

Diabetes Assessment for patients aged 40-49

  • Assessing the patient’s ‘high risk’ score using the Australian Type 2 Diabetes Risk Assessment Tool.

Flu Vaccinations

  • Flu vaccines are available at our clinic and are typically administered by our nurses in coordination with GPs consulting from our practice.

    Eligible patients may receive the vaccine free of charge under the National Immunisation Program.

    You may qualify if you are:

    • A child aged 6 months to 5 years
    • An adult aged 65 and over
    • Someone with a chronic health condition

Shingles Vaccinations

Patients in the following age groups are eligible for a bulk-billed 2-dose program of the Shingles vaccine once every 5 years:

  • Australians aged 65 years and older.
  • Individuals aged 19 years and above who have or will have a weakened immune system.
