Chronic Disease Management (CDM) plans are designed to help patients living with long- term health conditions better manage their care and stay on top of their health goals.
GPs consulting from our clinic can work with eligible patients to create personalised care plans tailored to their individual needs. These plans may also include referrals to Allied Health Practitioners such as physiotherapists, podiatrists, dietitians, or psychologists — with some sessions potentially covered by Medicare.
Benefits of a CDM Plan:
If you’re living with a chronic illness and want to explore your options, speak to your GP about whether you’re eligible for a care plan.
Additionally, patients may have the opportunity to attend sessions with Allied Health Practitioners (physio, podiatry, osteo, psychology and etc), which may be covered by Medicare, further enhancing their access to comprehensive care and support.
